I pray that this finds you well, and in the best of health and spirits. May Allah grant you all good and success in this life and the next.
The important thing is to deal with such an individual with the mercy, gentleness, and love that are the very essence of the sunna. After all, Allah Most High himself tells the Prophet (peace and blessings be upon him) in the Qur'an, "Verily, We have only sent you as a mercy for all creation."
Thus, the local community should be made aware of the special needs of disabled Muslims, and learn how to deal with them with the proper sensitivity and awareness. The onus would be on the imam and those who are active in the mosque to take care of this.
With that, the guardians of such Muslims should take the means to ensure that they don't cause an undue nuisance during the prayer. If they do so, then their guardian (or the person taking care of them) should break their prayer, and take the means to avoid undue disturbance.
The hadiths about keeping the mentally challenged from the mosque refers to situations when their behavior cannot normally be controlled and it is reasonably feared that they would cause undue nuisance.
And Allah alone gives success.
